Humans With Stuff On Their Heads
I would like some realism, please. When Romulans speak to each other, they should speak Romulan and the scene should be subtitled. I don't care if the mass viewing audience doesn't want to read stuff at the bottom of the screen. The universal translator should sometimes mis-translate. And I really don't buy this Earth-parallel development crap or how the Preservers seeded the galaxy and there are many humanoid species that, of course, look exactly like humans and speak perfect English. It just wreaks of "low budget."
The eleventh Star Trek movie did much better in making aliens look non-human, but it doesn't make up for the rest of the series. Let's go through some examples, and I'm going to skip a lot of "aliens" that they didn't even bother making non-human to begin with:

Unless these are all examples of aliens who are part offspring from humans, there's something in the back of my mind that keeps nagging me that these are just humans in make-up. If Paramount is too cheap to fund some realistic, completely non-humanoid prosthetics, they need to hold some bake sales. Trekkies are always willing to pay top dollar for cupcakes with the delta shield logo on them.
